Blue Cross Blue Shield does not provide Subscriber ID cards. When you are ready to receive covered services, your provider can verify your coverage with Blue Cross Blue Shield by using your Social Security Number. You can print a temporary ID card to present by logging into the BCBS website.

Preventive care pays off!

Your preventive visit does not count toward your calendar year maximum. Because the maximum applies only to non-preventive care, you can stretch your benefits further and pay less out of pocket overall.

Use Your Dental Benefits Wisely

Here’s how to make the most of your dental benefits:

  • Choose a provider — Using a participating dentist in the BCBS network will ensure you keep your costs as low as possible. Out-of-network providers may charge above the plan’s approved rates. To find a network provider, visit the BCBS website.
  • If your service will exceed $300, submit for a pretreatment estimate. You should always submit a request for a pretreatment estimate for procedures and services your dentist believes will exceed $300 (procedures such as crowns, inlays, bridges, and periodontics). Simply have your dentist submit a request online. You and your dentist will receive an estimate for most procedures while you are still in the office. Actual payments may vary depending upon plan maximums, deductibles, frequency limits, and other conditions at time of payment.
  • Check your claim status and other information on the BCBS website. You can review Explanation of Benefits (EOB) statements, check if claims have been paid, and more.
